- You will need fill out your details on the form before attending (instructions are in the email that you were sent)
They don’t accept the form you gave me
- You will need to make another booking (if you are no longer at the clinic), let them know that the form is fine and if they have any further issues to call us.
- Confirm with us which clinic it was. We can call them to confirm the form details and billing codes if there is any confusion.
